Scope of This Privacy Policy


This Privacy Policy applies to your use of PGL + PCL’s public facing website, www.providerschoicelab.com. This privacy statement also applies to information shared with us by our approved partners and other third-party sources.
This Privacy Policy does not apply to information collected through other means, such as by telephone or in person. This Privacy Policy does not apply to information collected by any third party that may link to or be accessible from or on the Site.

We do not collect precise information (e.g., GPS data, latitude and longitude) concerning the location from which you access the Services, but we may collect information on your region and postal code to help us gather information useful for improving the relevance of our content and securing our Services. 

 


SMS Data


When you provide your phone number and consent to receive SMS communications from us, we may contact you via text message for customer service interactions (e.g., supply shipments or courier pickups) or to update you on our laboratory schedule for holidays or in case of inclement weather. SMS consent is not shared with third parties for marketing purposes. 

 


Advertising


We do not use third party advertising to serve ads to you when you access our Services.

HIPAA Policies


If you are one of our patients, some of your personal information in our possession is protected health information (“PHI”) protected by the Health Insurance Portability and Accountability Act of 1996, as amended (“HIPAA”), and the applicable provisions of the Health Information Technology for Economic and Clinical Health (“HITECH”) Act. In addition to this Privacy Policy, our HIPAA Notices of Privacy Practices apply to your PHI. 


In collecting Personal Information through our Site, we may also collect PHI. Just as we strive to protect Personal Information, we are committed to protecting your PHI. If there is a breach of your PHI, we are required by law to notify you. Your PHI will only be disclosed to you or your personal representative, unless otherwise required or permitted by law. 

 


Protecting Children’s Privacy


We are committed to protecting children’s privacy on the internet, and we do not knowingly collect personal information from children under the age of 13. We do not knowingly allow children under the age of 13 to communicate with us or interact with our Site.
