Introduction

By providing your phone number and consent to receive SMS communications from Physicians Group Laboratories, LLC and Providers’ Choice Laboratory (collectively, “PGL”), you agree to the following terms and conditions. These terms outline how we use your phone number, the types of communications you can expect, and how to manage your preferences.

1. Consent for SMS Communications
Information (phone numbers) obtained as part of the SMS consent process will never be shared with third parties or affiliates for marketing purposes.

2. Types of SMS Communications
If consent has been given to receive text messages from PGL, messages may be received related to the following:
●    Specimen Pick Ups
●    Supply Orders
●    Inclement Weather Closures
●    Follow-up messages

Examples: “We have confirmed your specimen pick up for today, February 12th. You confirmation number is ABC123.”
“For the safety of our employees, PGL will be closed on October 1 due to inclement weather in the Houma area. We plan to reopen on October 2.” 

We do not send promotional or marketing messages via text.

3. Message Frequency:
The number of messages you receive may vary depending on the type of communication. For example, up to 3 SMS messages per week may be received related to supply orders, specimen pickups, and facility closures due to inclement weather. 

4. Potential Fees for SMS Messaging:
Standard message and data rates may apply based on your mobile carrier’s pricing plan. These fees may vary if the message is sent domestically or internationally.

5. Opt-In Method:
Opt-in to receive SMS messages from PGL by contacting PGL’s Customer Experience Department at (855)482-3598.

6. Opt-Out Method: 
To stop receiving SMS communications at any time, reply “STOP” to any SMS message received. You will receive a confirmation of your opt-out request, and no further messages will be sent unless you provide consent again.
Alternatively, you may contact us directly at (855)482-3598 to request removal from the messaging list.
